    Boeing crash in Indonesia a result of lax safety, faulty systems

    POOR safety procedures and a struggle by the pilots to control a repeatedly malfunctioning Boeing 737 MAX may have contributed to the crash of Lion Air flight JT610 that killed 189 people, reports Bloomberg,

    According to a preliminary investigation report, during the 11-minute flight before the crash in the Java Sea, the pilots battled with an automatic safety system that was relying on faulty instruments that hadn't been properly fixed despite similar failures on the plane's previous flight from Bali. 

    The report from Indonesia's National Transportation Safety Committee (NTSC), based on information from the plane's "black box" data recorder, ATC records, interviews with prior crews and other logs

    The report also criticised Jakarta-based Lion Air's safety culture and said pilots must be allowed to decide if they want to continue a flight.

    Boeing, the maker of the plane, said the 737 MAX series of jets are "as safe as any airplane that has ever flown".

    "Both the flights, from Bali to Jakarta and the final trip, suffered from different airspeed, altitude and angle of attack readings," said NTSC lead investigator Nurcahyo Utomo. 

    Mr Utomo said the pilots on the flight from Bali the previous day should not have flown the plane to Jakarta, but should have turned back.
